Only 18.2% of patients get there
The 2026 GOLD Report recommends a follow-up visit within one to four weeks of a COPD exacerbation, yet only 18.2% of discharged patients see a pulmonologist inside that window. Up to half are readmitted within thirty days. If the ED notes the need for follow-up with a respirologist, the patient may still be lost to follow-up if no one owns the hand-off.
