A Viz.ai Report: How Health Systems Are Closing the Gap Between Diagnosis and Care

The biggest threat to a patient’s outcome usually isn’t a missing treatment. It’s the handoff. The moment between when a care team spots a problem and when the right specialist actually sees the patient is where care breaks down, and it breaks down the same way across specialties: no reliable system flags the patient, no one owns the follow-up, and the window to intervene closes before anyone acts.

This report looks at three of those handoffs across three different care pathways: COPD patients discharged without pulmonology follow-up, cryptogenic stroke patients who need a cardiology consult before they leave the hospital, and oncology patients whose care teams are working from an incomplete picture of their own chart.

Download the report to see what happens when that gap gets closed systematically, in the words of the clinicians doing the work.
